Origin Theme - Custom Contact Form, Automatic link product name from product

Hi,

I am trying to have the product name to be automatically filled out on the contact form when people click the link in the product page so they don't need to type the product name themselves. Can someone please help me with this? Please see the ref image below.

@devcoders I think there was some domain transfer issue as I change the store link overnight.
