Popup For some category products

My online store name is MEDILAND PHARMACY. This store on Dawn theme.

Popups need to appear for some particular category product. Collected data need to save to database and we need to get email notification, while click on the Add to cart button the popup will comes, like the attached image.  

The product name wants to change automatically for each popup.

Welcome to Shopify Community.

So this are additional fields that you want to add to gather information from the customer. So either you can use Product properties, where customer data will be saved in the order when created. or use contact form so you will get an email of the submission. 

Thus it needs custom coding so I would highly recommend you to seek help of a developer.
