Shopify app embed extension does not show for the app review theme

Hello Everyone,

We recently developed an app and submitted it for review after testing, however it kept getting rejected and finally suspended due to this:

The issue is, we have tested several times on our development stores and the theme embed Extension widget shows up and works fine, we even shared a screencast of this, however the shopify app review team do not see the extension on their end.

As you can see below the embed widget works fine on development test stores

However the shopify review team cannot see the embed widget on their stores, we provided them with screencasts but to no avail. Any assistance on how to debug, test or even contact the review team about this would be greatly appreciated.
