Thumbnail image for product not showing in packaging slips

For some reason the thumbnail for the product image is no longer showing in the packaging slip. I tried to revert to the standard one (no mods) and also the problem persist. Not sure if this is a temporary glitch or what? Been like this for the past 2 or 3 days.
