Adding flodesk inline form to craft theme

Hello everyone. I've added a flodesk inline form into my website and it is appearing on each page as I hoped. However the form isn't filling out the width of whole page when I go to view the website (have attached a screen shot of what it looks like). I also need to have no padding between that block and the bottom block but I can't remove it. 

Have also attached a pic of where I uploaded the code into.

The code isn't actually working at all anymore. 

The flodesk form tells me to embed these two codes (in attached pic). 

Could you tell me exactly where to put these in craft theme?
