Charging additional fee for extra quantity

Hi all, thanks in advance for your help. 

We're selling one type of translation service (let's say $99 per page) and then add $49 for additional page. 

Since we're selling only one product (or service), I want to have it as main product with the option for customers to add based on the number of pages they have. Also, we want to add some questions on their preference and then upload their documents. 

Is there an app that we can do? I tried it with product customizer, but couldn't replicate what I wanted to do. 

Please let me know if you've seen any site providing or know how to get it done! Much appreciated.

You can use a product options app, to create an upsell for that product.

King Product Options has the ability to offer a text field, and upload section for your customers, as well as product add-ons. You can contact them directly to share your use case and they can help you set the app up.  

Globo Product Options also offers the same features and even has conditional logic, so the fields can change depending on what option is selected by the customer.
