Custom quantity and subscriptions

I am trying to sell a small handful of products (4-5) where the only thing that changes is the quantity and price for each customer.  The customer would answer a handful of questions about their situation and I would like to calculate the monthly quantity and price for their subscription.  However, base products would be the same across customers.  

Any tips or apps to implement this?

This is an interesting request. Off the top of my head, I know that it is VERY possible to set up automatic discounts for specific customer segments using metafield values. That's not the issue; I'm wondering how the quantity of orders gets configured on the product side.

Do you have any stores that you're benchmarking? Does anyone you know of do something similar?

I do not know of anyone specific doing it on Shopify.  However, you would see this with products like meal delivery kit businesses (e.g. Blue Apron, Home Chef) where you would set size of order, number of people, number of days, and what type of food you wanted before you got to a subscription price.  I assume that custom fertilizer company would have to get location, yard size, etc... to customize their subscriptions.  

I need to be able to take a handful of customer responses, then calculate a recommended order size for subscription (e.g. 15 widgets per month).  If it can go through and calculate and price the subscription automatically, that would be perfect.  If I had to start with the customer selecting the quantity manually based on the calculated outputs from the questions above, that would be an acceptable short term work around.  I also need to collect the additional customer information on product mix selection for fulfillment and store the responses to the questions.

This sounds amazing, and I think it is possible. I feel like you're heading towards a custom build with several moving parts or an app stack that gets it all done.
