How can I add a recipe request form to my online store?

How do I Create a form on my Shopify store where customers can submit their recipe requests. Include fields for the recipe name, ingredients, instructions, and any other relevant details. I could also ask for a photo of the finished dish

Roughly create an alternate template from the current contact form template and add new fields.

Shopify themes contact form api does not support file uploads at this time and so far looks like it never will. The closest native  clunky workaround to do this is to make the person go through the checkout process with a free product using line-item-properties to capture a file upload.

Otherwise you would need to implement some sort of custom image hosting,  or use form app or form service like google forms ,  or a scripting app to try and build it. Research "shopify contact form uploads".

Instead ask for the public url to that product, emphasize the public in the labels and messaging as a lot of people will mindless share a link to something only they can use.

Would be dope to have recipe submissions so food lovers can sell recipes and products and build community and not have to run to wordpress. 

Truth is many don't want to use wordpress because they have no support. if shopify upgrade their blog they could be a complete shoping solutions. remember people buy ingredients and community is what leads to more sales. expand shopify... while keeping what's it's all about.
