How to add a custom gift message option to product prices?

Hi, I'd like to add an option for customers to send a Custom Gift Message to be sent to the recipient of their gift product. 

i.e.

On current product page - $79.99

Add Custom Gift Message (+$2.99):

Any ideas on how to execute? We'd like the price to be added to their current product price if they select the option to send a gift message.

Offering gift messages as add-ons is a great selling point.
