How to add an input field ''Name'' to the email signup section in footer in my Dawn Theme.

Hi team,

I really need to add the ''Name'' field on top of the ''Email'' field as this is such important information to collect for my email list. 

Will much appreciate it if you can give me a step-by-step of what the code is and which section should I add it.

you have replied saying thanks and it was simple to add the name filed into your email sign up. Do you have the fix for it, as there isn't any information on here now?
