How to add extra field at checkout to collect ETH address

I want to sell NFTs by selling products so i need a field where customer can add additional information before checking out. I be want to add field so user can optionally add their ETH address to be included on receipt for a record and then i can just manually send them them the art to their address. I also need to add special instructions of what to do to see their nft. The added field could be near the discount code since address section isnt used when using apple pay.
