I do a ton of custom work for customers - using their designs as the basis for the products I sell. I don't want to create products in my database for THEIR art, but need to be able to invoice customers for the work I do - and I'd like to be able to add a custom image (usually the digital mockup they have approved) to the invoice so they can see what they're being billed for. Is there a way to do this? I find it odd that I can't even have my shop logo... it's just a grey box with a generic camera icon for an image and looks horribly unprofessional - like I didn't take the time to upload an image, which the camera icon implies SHOULD be there.

I totally understand the need for a feature like this, though at this time, images cannot be added directly to draft orders. Luckily, I have a workaround that can solve both of the issues you outlined.
As far as the camera icon, if you are linking a custom product, you can always create a product specifically for your custom work and add your logo as the image. Then when creating your drafts, simply select that product (instead of the add custom product button) and you'll be able to generate a draft order that eliminates the generic camera icon.
As for being able to provide a digital mockup, the best way to do this is to upload the image to your Files section (or your desired third-party image hosting site). Then, create your draft order and include the image link using HTML in the email body field of your invoice preview.
Upon clicking "review" you will be able to see a preview of your mockup right in the email body of the invoice to your customer. Let me know what you think of those options.
I love that you are in the business of custom work! In case you weren't aware, Shopify recently added a new feature which enables you to add up to 2 business days of processing time when customers are checking out. If you have some products set-up for direct purchase - this might be a beneficial feature to add.

I really this was possible to add an image to a custom product in a draft order. I also do a lot of one off custom orders and really don't want to create a product for every one, but the customer needs to see what they are purchasing
I also want this solution! I don't like creating a product for every custom order. It takes too much time to add all the product fields for a custom item that will never show up on my website. The customer just needs to see an image of what they are purchasing, to confirm the design is correct, and then buy it.

I have a plausible workaround for this, I have just tested and managed to get the image to show on the draft order invoice email template, the packing slips , and also the "Print Order" (the Shopify native ones) on the order page.
Disclaimer: I am the developer of the Draft Order Helper app (https://apps.shopify.com/draft-helper) , which the following step will utilize a feature from this app to add the image link to the custom item in the draft order, the app has a free plan which you can test it out.
Before adding the image to the custom line item, you can upload the image into your Store > Files , and copy the link to the image.
Then you can install my app (Draft Order Helper), then go to your draft order, then click "More Actions" > "Edit line item properties"
Then click "Edit Properties" for the custom product, and paste the copied image link into the value, and use "image_url" for the name :
Click "Apply changes" :
Now you should see the custom image on the draft order!
When a customer checkout the draft order, they can click on the "image_url" link to view the image (unfortunately the image will not show directly, customer have to click the link)
After that, you can customize the Packing slip template code (I can help you with this after you have installed my app), to include the code to show the image_url value as image.
And the packing slip will looks like this (with the custom image) :
You can also customize the draft order invoice template code :
And then in the invoice email, customer can see the custom product image :
