How to implement a personalized skincare quiz on a Shopify site?

Hi! I'm looking to create an experience where a customer will go through a quiz which includes uploading a photo of themselves and answering a few questions about their skin. Once all the steps of the quiz are complete, a skincare specialist reviews the photos and the answers, and then the skincare specialist picks the right 3 products for that specific customer (so it isn't an automated picking process). Once the skincare specialist picks the right three products, the customer can shop those three products only. 

I've already spoken to companies such as Jebbit and they do not offer such capabilities.

Any ideas on how this can be executed using a Shopify based site (of course this can be done through a custom JS site but that's significantly more time and $$)?
