Interactive quizz/form/questionnaire

Is there an app that lets me create an interactive quiz, survey, or questionnaire to collect and store customer information through a series of questions? Ideally, after the quiz or questionnaire is submitted, the system would triggered to send one of the pre-written emails with recommendations based on the customer's answers.

Thanks for your reply. Not so much a purchase survey. More like a questionnaire embedded on the site for customers to fill out, which will trigger a specific outcome/solution for the customer. Ideally, the system will send a particular pre-written email automatically post submission of the questionnaire based on the answers in the questionnaire.
