Looking for an app for a cart add on

Hi!

I'm looking to find an app that will let me add an option at check to add insurance (not through a provider, I will just collect this money and become my own insurance provider).

I'd like it to be a little box they tick off to include it and it will calculate the amount based on a % of the total cart value. It needs to be very obvious so it does not get missed over.

Does anyone know of an app that does this?
