You may be able to achieve this with the use of some third-party apps. Wholesale Lock Manager may be a good option as it enables you to use customer tags to lock and unlock specific products.
As far as needing to be able to renew after a year, it would be work looking into both Subscription based apps as well as Customer Account management apps.
Finding a solution with multiple apps is doable, but can be cumbersome to manage seamlessly and it may be worth bringing a Developer on board through our Shopify Experts to help build a custom solution for you.

Regarding:
"customers will have to register as patient, and fill a medical form. Based on the medical form customer have filled, he will be allowed to buy only certain product of this hidden collection, not all of them."
If this is not being done in a HIPAA-compliant way it's a pretty big violation. This information has to be stored outside of Shopify and encrypted to NIST standards.
Shopify can be made a part of a HIPAA-compliant solution so long as you are deidentifying and store PHI/ePHI outside of Shopify.
Name + prescription product purchased = non-compliant.

thank you, looking at your site. so if a patient purchases a medication on shopify, say progesterone. is that a hipaa violation? the actual prescription is sent in our EMR (legit, hipaa complaint, etc). all shopify would see is the customer info and what they bought. thank you
