Shopify Terminated My Store unable to & appeal

on appeal page my first name and last name are empty and i'm unable to write my name and how i contact

I'm having the same issue as hibaa.

I go to my email that says my account was terminated and then I click on the underlined "form" which sends me to the appeal page. Then I choose what store I want to appeal and am unable to fill in my name and last name, which is required to send the appeal.

My chrome is updated and I have cleared the cache on my browser. I have also tried to open the form on all my other devices, but the name and last name field won't allow me to input any information.

Hey, I don't know if you solved it but for you or anyone else who's having this issue, I believe I found the solution. If you're having the issue of not being able to put in your last name, that's because you're not actually supposed to put it in right there. If you go to this link, you should be able to sign in and in the general settings you can change your last name and it will be updated on the dispute form allowing you to submit it. It took me hours to even find this link and it should not have been this hard, Shopify should definitely change something to fix this issue.
