Single one time product purchase with 3 subscription service options

We have a single physical product that we’d like to link to 3 possible subscription service options.

1) The customer can select 1 of 3 mandatory subscription (or membership) options (6, 12, or 24 months)

2) They pay for the physical product at checkout

3) The subscription/membership plan will start when they receive the product and activate it (we have the activation/notification part figured out on our end).

Essentially this would look similar to the checkout process for the Oura ring, Peloton, NordicTrack/iFit, etc.
