Terminated My Account 3 Days After Creation

Hi everyone. I just started my store and 3 days later, I received an email that my Shopify account got terminated for violating Shopify's Acceptable Use Policy. Our products are only simple clothing and we used the brand that we have copyright.

So I tried to appeal through the link from the email sent to me. I was asked to fill a form, but the form can't be filled because the First Name and Last Name Row are greyed out. I can't click the text box to fill in the form. Can someone help me offer a solution? Thank you very much.

I have not had this happen from Spotify but I did get the message from Tick Tock and Facebook and I dont know why?
