Urgent Help Required: Multi-Step Checkout With Subscriptions Needed

Hello shopify community,

I need your help and guide to achieve something the below website. I am using Dawn theme and Appstle app.

I want to know do I need to make a form for this?

- If yes, then how? Also, how form and Appstle will be connected? How it will be communicate and connected with checkout?

- If no, then what elements do i need to use for this? First, people need to select the cats options then they can select the monthly or yearly subscription option. How the subscription will be setup? Need to create a product for this? 

Please let me know how I can do it? If that is possible, then please mention all details or steps.
