Someone ordered a size, then messaged me on Instagram they want to do a different size. I had the order unfulfilled for about a week, then after I fulfilled it, the quantities were still all the same. Does it update the variant quantity as soon as the order is placed, before it is fulfilled?

Yes, the quantity will be deducted once the order is placed in your Shopify store. If an order is unfulfilled then the products included in the order will have the committed inventory which needs to be fulfilled.
If you check the image below, at initial product variant quantity is set to 447 then we placed a test order with this product and didn't fulfill the order so it shows the committed quantity(which needs to be fulfilled) as 1 and will deduct the available inventory.
