Custom Order Form

I am a Tailor making custom garments.

I want to make an order form where people can measure their existing garment and enter measurements into fields. I need to be able to put a photo of how to take each measurement next to that specific measurement field.

Also need fields for other detail options such as collar type, buttons etc
