Help me with a script and solve my issue

How can I set up the checkout page so that the customer can choose a shipping address from a dropdown menu, showing only the addresses where the product is available and from where exactly he wants this product to be sent out? After the customer completes the payment, the chosen address should appear on the Thank You page.
