Is adding a checkbox to the checkout page possible on Shopify Plus?

Hello,

We are using Shopify Plus and wanted to know whether it is possible to add a "checkbox" to the checkout page?

Essentially we wanted the checkbox to allow our users to self-identify as a business or individual (simple yes or no), and we would need that the answer of the checkbox shows up in the order's details.

Any help would be greatly appreciated,
