Add a input box in checkout or in the cart page where customers can input their Sales Rep name

Is there a way to add an input box in checkout or in the cart page where customers can input their Sales Rep name and this gets added to the Customer account as a note?

Hi @j_adv21 , notes that you include on cart page will be saved as order notes, not customer notes. Which theme are you using?
