ADD A LINK to a (POP UP or PAGE) ON A SPECIFIC PRODUCT WHEN A CLIENT ADD TO CART

Hello there!

I want to add a certain link to a pop up or another page on product when a customer adds the product to cart.
This is just for a specific product not all! I have this custom product and i want a customer to fill a form when they add to cart.

Can someone out here help me please?
