I'm looking to set up a dealer application form to our login page. The attached pictures show what the form looks like on the current website. I want to duplicate that under the sign in button on our log in page. If it has to be a link to another page that is fine but I want to avoid using an app if possible. Has anyone found a solution for what we want to accomplish? We need sections for their name, email, business name, and a way to upload forms to the website.

You can create a snippet, place the form code there, and call the snippet in the log in page.
Where is the form located? It would be best to see your website, you can also put a small button to link them to that form you place
I think if you want to go easy you should go with any Free/Paid custom registration form apps. I would not say it is impossible to code a custom signup form within the theme but it is complex.
If your requirement only had their name, email, business name I would have suggested to go without app. But Document upload is tricky one. You need to store documents on a server. You may need to purchase a secure hosting.
Instead if you go with form apps they do the work. You can build custom registration form with few easy steps. You can add/edit whatever you want and whenever you want (with secure file upload).
