Display 2nd newsletter form on a specific single "page" (besides the footer newsletter form)

Hey guys, I am struggling right now to implement a second newsletter form on a single page. 

1st problem: I am lacking the code to paste into either the rich text editor of the specific page, or the code for the theme file (creating new template for page where newsletter code is inserted)

2nd problem: The newsletter form on the lets call it landing page, should be a different mailing list, than the usual newsletter form from the footer menu of my Debut theme. 

Any ideas on how to solve this?

Did you ever find a solution? 
I am trying to do something similar. However, I want them to go to the same list.
