We sell a technical product as a single SKU that is comprised of multiple installation components. When we receive a new order, we send customers a form to collect information on their specific installation, which helps us determine which installation components to ship with the order. We do this instead of separating the installation components into additional SKUs to make the buying process easier for customers so that they do not have to understand all of the technical details of the product to get what they need.
Once we review the installation form, we want to mark some choices on the back end to provide to our order processing person. I am searching for a way to do this with simple check boxes instead of custom line item text. Does anyone know of a way to add selection criteria to the back end of an order?

You can custom code these form fields at product page and same as you send to customer post order. Answer chosen by user will be available as line-item in order along with product.
This will help you to remove the extra step of sending the form over the email.
