How to add mandatory check boxes on the cart page or check out page

I need to add mandatory checkboxes for my customers to either choose to opt-in or out of shipping insurance and/or signature confirmation. I know I can add shipping insurance apps that already allow the customer to choose to purchase shipping insurance but I want a mandatory checkbox they have to check if they choose not to purchase shipping insurance. I will attach a screenshot of an example of a website that has this and what I'm looking to incorporate on my website. The theme on my Shopify website is Impulse version 3.6.0. Any help or options would be appreciated, Thank you.

Hey Jambot this is what I ended up doing. I downloaded the Insureshield shipping insurance app by ups and they wrote me a code to integrate their app into my website.

These are the options you get for your customers to see in their cart when checking out. 

I made it so the customer has to uncheck the toggle if they opt out of the shipping insurance. I also included a disclaimer by modifying the shipping insurance title by finding the insureshield insurance on my products page within my store. I will insert a picture of what that looks like in my cart. Hopefully, this helps it's the best I could figure out on my own.
