Product listed on invoice that we do not carry and have never carried.

We just printed an invoice and the last item listed was for a gold chain for $647. That $647 was not a added to the final total that the customer paid.  We sell face paint, not jewelry, so we reprinted the invoice and the product did not show up. 
How in the world could this have happened?
