Text in Required Field on a product page suddenly not coming through on the order.

Hello, I submitted code into a product-form2.liquid product page for there to be a text box that is required to have text in it before the product can be added to the cart.  There are only 1 out of every 10 orders now where the text suddenly is not coming through on the final order, therefore the "required text" is vanishing. The customers are 100% typing the information in and I can't pinpoint what suddenly started causing this over the past 2-3 weeks.

this has been happening to me, too. every 7 or so orders, the text field does not show on orders page. my customers are also inputting information. any luck resolving the issue?
