When is a FulfillmentOrder created?

Sometimes, for an Order LineItem, such as a Tip, a FulfillmentOrder is not created.

Other times, for a Commission, a FulfillmentOrder is created. It is created even when product_exists is false and requires_shipping is false and the order was from POS.
