Why are two forms loading simultaneously on my website?

Hello everyone,

I hope someone can help me on that matter. 

I have an issue with my notify-me form on my shopify website. Indeed, as you might see on this page (https://rougeabsolu.com/collections/mobiliers/products/bibliotheque-up-to-you), a first form is loading then a second one is.

My issue is that I want only the first form and I wish that he wasn't replaced by the second one.
