Why is my intake form submit button not working on Advanced Shopify?

People have been telling us that our intake form submit button is not working. It was working before. Nothing happens when you fill out the form and click the submit button. We are on the Advanced Shopify plan. Any ideas for how to fix this would be much appreciated. Thanks!
