Why isn't my contact us page displaying correctly?

Hi, Can someone please help me out on this? 

My contact page is not showing up. It shows the header and footer alright but the page content (the forms and text fields) are not showing.

The correct template to choose is labelled contact as seen below, so you are choosing the correct one.
