Had the same issue. `shopify app connect` command fixed the issue.

Just add middleware which populates required header. It worked for me.
I am using `fulfillmentTrackingInfoUpdateV2` mutation to update tracking info. The problem that I need to refresh the page for tracking number to appear on the UI. How can I refresh page/card programatically?
