Thank you for your comments. Problem solved: The reason the link returned a 404 error is because the text "pages/" needs to appear in the URL before the page name (pages/wholesale-signup).

Figured it out. The text "pages/" needs to appear before the page name (pages/wholesale-signup). I'm not a techie. Why Shopify tech support could not figure that out is beyond me.

Thank you for offering JsRates to display expected shipping date. At this time however, until we generate sufficient sales, our shop is limited to the "Basic" Shopify plan. For that reason we are prevented from testing the JsRates app.
