Thats what im having trouble with right now. What do I need to modify to fix the problem

How do I make the word tax id on the right of the of the box and the send button doesnt work so how can I fix that as well. If anyone can give me a new code for everything with this similar layout that would be great as well.

I want it to look like this or something similar to it with more bubbles asking for address information as well
