I found an app called Releaseit Cash On Delivery that lets me choose forwhich country, type of customers, order value etc to allow this paymentmethod (which was basically what I wanted).

I have the same question. For instance, I would like that my manual COD payment option is available only for orders over a certain value. Any way to do this?
