A form is mysteriously appearing at the bottom of every page.

At the very bottom of every page of my site, www.carolynkdesigns.com is a form that should not be there. It’s under the footer and copyright note. This is the same form I use to sign up people for my email list but they should only be able to access it through the link in the footer that says “Join VIP Club”. I can’t find the code to delete this. When I search for code, nothing happens, even when I search for words that I know are there.

Something is referencing this for every page. When I go to edit my theme (Dawn 2.0), it can’t be edited. It’s there under the footer, but I can’t see how to delete it.

Does anyone know how to remove this from each page?

And the form is coming only when I click JOIN VIP CLUB link.
