I see what you mean. However, to provide the best possible option, we’d need to understand the following steps in the process. Once the customer submits the form, how do you see the approval process happening? Also, let’s say the customer is eligible for the product upon review, how are they notified about it, and how do they proceed to complete the purchase?
Considering it seems like a complex logistical process, the best option is to get the custom app built. If you need to outsource help on it, you could post a job on the Shopify Experts Page and see what feedback and quotes you get.
Otherwise, you could combine custom product templates and draft orders. First, you would create a separate product template which you will assign only to specific products, such as prescription products like in this case. This template will trigger a form you create once the customer clicks the button. Once the form is submitted, you should receive it in your store email. Next, if the order is approved, you would make a draft order by inputting the product and customer information. Sent the request for payment to the customer’s email. Once they complete the purchase, you’re left with the order to fulfill.
To start this setup, you need a form builder app such as Easy Contact Form Builder. First, opt for a Pop-Up form in the Display section. Then, in the Trigger Button section, select Embed to your page content. Next, you can edit the Fields section to your liking. Finally, follow the instructions for embedding this button to the page, as shown in the screenshot below. Also, in this case, you can hide the Add to Cart button from that product page template by clicking the eye icon in the Theme Editor.
Now you have to create a custom product template. To do that, ensure you are using an Online Store 2.0. theme. Then, follow these steps to set up a new template. Once the template is done, you can add the Custom liquid block and paste that button code from the app. Don’t forget to save changes in the Theme Editor.
While this option sounds time-consuming and not ideal, I wanted to share as many options as possible so you can see what works best for you. Let me know what you think.
