Added custom fields to contact form, but not getting the fields in submissions

Topic summary

A user added custom fields to their Shopify contact form that display correctly on the website but don’t appear in email submissions when the form is submitted.

Problem Details:

  • Custom fields (date, company, and other fields) show on the live form at the website URL
  • Only one field (company) was initially appearing in submission emails
  • The remaining custom fields were missing from email notifications

Solution:

  • A community member provided the fix: properly format input fields using the syntax <input name="contact[company]">
  • This resolved the issue after the user had spent 5+ hours troubleshooting

Status: Resolved. The correct input name formatting allowed all custom fields to appear in form submissions.

Hi everyone,

I updated my contact form with some additional fields by adding the code, it shows up great when you go to my website and view the form: https://www.westmichiganballoons.com/pages/general-inquiries (I’m actively updating the live one this AM so if it looks wonky when you look it’s likely I’m messing with it)

However, when I submit a form, I don’t get those fields in my email. I was able to add one of the fields, company, and that showed up, but the date, the other field I added, etc, don’t show up when the form is submitted. Any ideas?? Thank you!
