Adding a survey/questionnaire at the checkout page on the dawn theme

I’m just wondering how to add a few questions like allergies just before someone purchases an item and would like this to be done on the checkout page. Let me know if someone can help me.

Unfortunately Checkout pages can’t be altered, unless you’re on a Shopify plus plan.

Alternatively you could put that survey on the CART page.
