Advanced form - sending product information to me

Hi,

I need an advanced form so my customer could sent me all information about products they want to sell throuth my ecommerce platform (including upload section).

I would like this form nearly like this: https://www.aptdeco.com/sell/new

Any advise for an app?

Hey Vincent this may not be exactly what the example was, but have you tried this app https://apps.shopify.com/uploadery or similar ones?

I looked at many apps, but for now I didn’t find what could fit for me!

It appears that site is using the Go-Daddy web builder. I have used it before.
