Thank you for the screenshot! To confirm, are you the merchant running this store and you are having issues with your contact us form? If yes, can you please provide a link to the contact form page and some samples of the error(s) you are seeing?
If you purchased an item that you believe to be from a Shopify merchant I would try emailing them directly or replying back to your order confirmation email with any questions you might have.
