Hi, I’m looking for an app or way to add custom text to a clothing product for a fee. Basically, I’d like to sell the blank product at X cost, and give the customer to add their name or initials to the product for a small fee, say $5.
All of the personalization/customization apps I’ve found don’t add to a blank product (i.e. they have pre-existing designs on shirts, mugs, etc that you can change the text on). I need something that will both allow me to sell the blank item and have the option to add a monogram, initials, name, etc. for a small fee.
Please advise if any apps exist, thanks!

As I understand it, you’d like one app that will both accept customer input and also apply this input towards customising their product.
I’m not aware of one solution that would combine both of these steps, there are apps like Product Options & Customizer that allow for customer input and for pricing to be charged based on customisation.
Are you looking to add personalisation to products you source yourself or are you looking for a service that provides the printing as well?

Both of apps allow you to add text inputs into your product image and customize the text to upcharge on it.
Product Options and Customizer has free plan forever while Zepto Product Personalizer require a $9.99 month fee after 15-day trial.
Just try both of them and find the app you prefer to.

You can consider the Infinite Options app by ShopPad Inc. It gives the flexibility to add unlimited custom product options including text, numbers, calendars, swatches, checkboxes, and dropdown menus. It also gives an option to personalize products for engraving, or monogramming.
Besides, there is always an option to develop a custom Shopify app tailored exactly to the needs and visual styling of your store.
Add personalization to existing products - we already work with a printing service that would need to integrate with this app
We tried InkyBay and it works great on the front end but doesn’t integrate with our printers, so we couldn’t actually make any product out of it
