Best way to attach an add-on to a product without creating it as a separate product?

Is there a way or an app that allows me to attach an add-on to a product without having to create that add-on as a product? Also without creating it as an option within the product (since my product already has 40 variants, so adding another option for the add-on would double those variants).

Use case: I sell a product with 40 different possible variants. I want to offer the user the ability to add insurance on their product (maybe as a popup after they click add to cart or it could show in the product page above the add to cart button for the user to select the insurance option).

What is the best way to achieve this on Shopify?
