Is there a way to have a buy X get Y promotion, without customers having to add Y to the cart?

Really great question. Shopify’s automatic buy x get y discount doesn’t quite have this feature, so you’ll need to look elsewhere to get this, such as an app or have something custom made by Shopify Experts.
There are several buy x get y apps in the Shopify App store which you can see here. I looked at some of them and none outright say in the description about the ‘y’ being added automatically. It could be worth researching and finding one you resonate with.
Last but not least, if you can’t find an app to do this, it might require some custom work. For this, I’d recommend a Shopify Expert from the Expert directory to work with on it.
You can consider setting up a product kit of X + Y at a discounted price. Though it’s not exactly the same promotion as BXGY

If you want gifts to be automatically added to the cart, the best solution is to use a third-party app like BOGOS App, because Shopify’s native discount feature doesn’t support this functionality.

No, there is no way to add a product automatically to the cart using the native Shopify discounts. Shopify Buy X Get Y discount requires the customers to manually add the product Y to the cart.
However, you can explore various free gifting apps from the Shopify App Store. I recommend you try Monk, it not only adds the product automatically to the cart, when ineligible it automatically removes the product from the cart as well.
You can also create highly targeted offers by combining various eligibility rules like, item quantity, cart value, specific product, specific collection, customer tag etc. or a progress bar to incentivize for higher cart value.

Yes, but usually Shopify’s built-in discounts require the customer to manually add the Y product to their cart. For auto-add the free product, several merchants try EasyGift, Monk Free Gift or BOGOS. These apps add the free product automatically once you add the qualifying product, which creates a much better conversion flow.
If you are a developer and familiar with theme development, you could also do this with Shopify Functions + cart AJAX on supported plans, but for the majority of stores an app is the faster, easier and more reliable approach.
