does shopify have any capability to allow customers (shoppers) to personalise/customise product designs? For example if I was selling birthday cards using a print on demand fulfilment app, could I allow my customers to add a personalised message in the card?

Depending on what format you want to allow your customers to upload, there are a few ways to achieve this.
First, you can collect product customisation information using line item properties on product pages. We have a great guide on implementing this feature on this link. This is an advanced tutorial and is not supported by Shopify. Knowledge of HTML, CSS, JavaScript, and Liquid is required. If you don’t feel comfortable doing it yourself, please consider hiring a Shopify Experts.
